Final Diagnosis For Which Treatment Was Sought Including Patient's Actual Condition
Patient presented to Dr. Baker?s office with low back pain radiating into the right groin and left hip. An abnormal MRI reviewed by the referring physician revealed a large, dumbbell shaped tumor emanating from the neural foramen at the left L2-3 area. Dr. Baker?s assessment was that the tumor had destroyed portions of the vertebral body of L2 and the exiting nerve root. Treatment required removal of the tumor and stabilization of the L2-L3 level. Patient decided to undergo resection of the tumor, a decompression laminectomy at L2-L3, and L4 and a pedicle instrumented fusion. The surgery was completed without complication and patient was discharged home.
Operation, Diagnostic, Or Treatment Procedure Rendered Causing The Injury
Following the surgery problems with infection arose and patient underwent surgical debridements on 2/15/05, 7/15/05 and /6/05. The first and third debridements were performed by Dr. Baker, with the second performed by his partner Dr. Sawin.

The infection was consistently noted to be superficial, it never completely healed and eventually Dr. Baker recommended another surgery where new hardware would be placed to provide additional stability. Patient then sought a second opinion and on 1/26/2006 underwent surgery at Shands by another physician to remove the infected hardware. Patient then underwent a lengthy course of antibiotics and third surgery on 6/27/06 to remove more of the tumor. To date, patient is doing well and has not had any new/additional hardware placed in her lumbar region.
