Patient developed a post-op infection, which was treated initially with oral antibiotics.His condition did not improve, and he was admitted to the hospital for two weeks for IV antibiotic therapy and hyperbaric oxygen therapy.After discharge, he continued under insured?s care, and was treated with hyperbaric oxygen treatments, whirlpool and iodine treatments, which resolved his infection.Patient claims to have suffered an ischemic event during the surgery, leading to infection and gangrene.He alleges his past medical history contraindicated the use of an Epinephrine block; however, it should be noted that patient?s PCP testified that he medically cleared insured for surgery; the patient was pre-diabetic and not diabetic; and the patient never had evidence of circulatory or vascular problems before the surgery.The consulting infectious disease specialist had the opportunity to examine patient?s toes one week after the surgery and found no evidence of necrosis or ischemic changes.Instead, he opined that patient was suffering from a post-op infection. Likewise, the consulting vascular surgeon testified that based upon his consultation post-op, he found normal vascularity of patient?s right foot.Patient eventually healed the infection with use of the modalities referenced, all of which require blood perfusion to aid in healing. Had patient suffered an ischemic injury from the hammertoe surgery, these modalities would not have likely healed the infection.
