¿Symptommatic atrial flutter in an 87 yo male with ischemic heart disease, CHF, ICD, hx of atrial flutter and significant co-morbidities.He was scheduled for ablation.¿
Operation, Diagnostic, Or Treatment Procedure Rendered Causing The Injury
The patient underwent the ablation procedure June 18 2009 and the next day he was ambulating and requesting to be discharged to home.Dr. Annoni noted the patient¿s INR was 1.9 and subtherapeutic and had not yet reached 2.0 and as such, he held the patient¿s discharge. The next night following the patient straining during a bowel movement, a smallhematoma was observed and the nurse notified Dr. Annoni who ordered that pressure be applied and ice packs.The next morning when Dr. Annoni did rounds he observed the area to be bleeding, which was more significant than what nurses told him the night before.Dr. Annoni ordered a stat consult with a vascular surgeon who took the patient to the OR wherehe repaired 2 small punctures to the femoral vein and femoral artery.The patient was taken back to surgery a second time later that day and the bleeding was noted to be in ¿the subcutaneous tissues¿. The patient was found to have a pseudo aneurysm and retroperitoneal hematoma.
