Final Diagnosis For Which Treatment Was Sought Including Patient's Actual Condition
Patient collapsed while working on his ranch. He was transported via ambulance to the ER where he was admitted with a differential diagnosis of stroke, TIA, heat stroke and or hypertension.
Operation, Diagnostic, Or Treatment Procedure Rendered Causing The Injury
A CT of the brain was interpreted as negative. The admitting doctor called for a consult with Dr. Kiss who was out of town that day. He agreed via cell phone with the admitting doctor to start the patient on Heparin and transfer to ICU. Dr. Kiss first saw the patient on 7-29-02, he had improved in the five hours he had been on Heparin and Dr. Kiss? impression was an acute left hemispheric transient ischemic CVA of uncertain etiology. He ordered continuation of Heparin and MRI. Patient worsened, Heparin was stopped and a STAT CT scan of the head was ordered which was negative. Dr. Kiss restarted the Heparin and reordered MRI scan for 7/31/02 first thing (this hospital only provides a mobile MRI unit so they had to wait until 7/31/02. A CT performed on 7/30/02 was positive showing a large right acute cerebellar infarct.

It was alleged that earlier treatment would have resulted in a better outcome than the late treatment received. More specific criticisms failure to ensure emergency coverage 24/7. It was also suggested that the patient should have been transferred based on the inability of this hospital to diagnose and treat a patient with obvious neurological signs. They were very critical in the delay to transfer patient prior to 7/31/02.

Permanent: Major - Paraplegia, blindness, loss of two limbs, brain damage.
