Diabetic patient was seen for the first and only time by insured on 5/15/10.The patient had been sent by her primary care provider with a diagnosis of an ulcer on the plantar aspect of the left foot under the second metatarsal.The patient had been placed on clindamycin by her PCP and was doing well with decreased erythema.It was noted that the patient had no palpable DP and PT pulses on the left foot.The ulcer was noted to be superficial with a small amount of pus.A culture was taken which grew methicillin susceptible Staphylococcus aureus sensitive to the clindamycin.The diagnosis was an ulcer of the ball of the left foot, IDDM and PVD.Treatment consisted of débridement of the ulceration.At this point it was noted that there was a ¿deep sinus tract that did not appear to reach bone¿. A dressing was applied and the patient was advised to return in one week.A letter was sent to the PCP and noninvasive vascular studies were recommended.The patient did not return.Rather, on 5/24 she was admitted to the hospital with a severe infection of the left foot that was documented as being a ¿gas gangrene¿.Interestingly, a CT scan showed no bony destruction.On 5/29 the patient underwent a below knee amputation.Patient alleges insured failed to immediately refer h for a vascular consult.Our expert is of the opinion that the patient¿s outcome would not have changed in this case had there been an immediate referral to a vascular surgeon.He believes insured continued patient on the appropriate antibiotic, and that her diabetes and non-compliance led to her infection and amputation.
