Patient on longstanding Coumadin presented with prostatitis for which he was prescribed Levaquin.The patient's insurance would not pay for the Levaquin, so Bactrim was substituted.

The patient was prescribed Bactrim while taking Coumadin and was informed of the risks of the combination of these medications and instructed the patient to obtain follow up labs so that his INR levels could be closely monitored.
