Final Diagnosis For Which Treatment Was Sought Including Patient's Actual Condition
The patient was suffering from rectal bleeding.He was to undergo an upper endoscopy for exploration and if that was negative, then a colonoscopy for further investigation.Upon Dr. Huh arrival it was noted that the patient had a drop in hematocrit and hemoglobin as well as a low blood pressure.Given these factors Dr. Huh found it incumbent to immediately undertake the endoscopy, since this was an emergent situation and a transfer to the ICU would have put the patient at more danger. After the procedure the patient was stable with normal blood pressure and pulse.Afterwards, the patient was rushed to intensive care were he was given two units of packed cells.His blood pressure continue to drop and two hours after the procedure he went into cardiac arrest.He was resuscitated but was noted to be extremely acidotic, despite efforts the patient continue to decline and his wife determined that in light of no positive results from continuing with the code that the efforts should be discontinued and Mr. Thompson expired at 8:58 a.m. on June 2, 2003.
Operation, Diagnostic, Or Treatment Procedure Rendered Causing The Injury
Patient underwent an emergent bedside endoscopy procedure. Dr. Huh performed the endoscopy procedure without sedation to avoid further lowering of the blood pressure.He performed the procedure at bedside with same monitoring as in the ICU.Dr. Huh resuscitated the patient with IV fluids while performing the procedure.When the procedure was completed the patient was noted to be stable with a normal blood pressure and pulse and was immediately transferred to ICU for close monitoring. Delay of the endoscopic procedure as well as taking the additional time to intubate the patient, secure the required equipment would have been too risky for this patient.

Dr. Huh conducted himself and evaluated the patient in an entirely appropriate manner.His decision to perform the endoscopic procedure at bedside was a good clinical jugment based on the circumstances.The endoscopy was clearly indicated, was performed and the fact that the patient suffered a cardiac arrest two hours after the pocedure has no casual connection to the location, method or manner in which the procedure was performed.
