On 6/7/11 and 11/8/13, patient underwent lab tests ordered by the insured as a result of high calcium levels. Results on both occasions indicated elevated calcium and parathyroid hormone levels. The patient was subsequently diagnosed with hyperparathyroidism and underwent partial removal of the parathyroid gland.
