Final Diagnosis For Which Treatment Was Sought Including Patient's Actual Condition
ALLEGED MISDIAGNOSIS OF A SPINAL CORD ABSCESS WHERE PATIENT IS NOW SEVERELY DISABLED AND UNABLE TO WORK.The pt complained of limited leg mobility, inability to urinate for 10 hrs, fever and upper back pain. Dr Diaz documented her impression that patient was suffering from acute febrile illness and alcohol withdrawal.
Operation, Diagnostic, Or Treatment Procedure Rendered Causing The Injury
Mr Keene presented to ER at FL Hospital East The pt complained of limited leg mobility, inability to urinate for 10 hrs, fever and upper back pain. Dr Diaz documented her impression that patient was suffering from acute febrile illness and alcohol withdrawal. Rq a CT scan of lumbar spine & brain Patient was noted to be suffering from excruciating back pain and he was catheterized. At that point he voided 2200 ml of urine Dr.Diaz called Dr St Louis. Our insured, Dean Wilson PA, was on call for his supervising neurosurgeon Dr St Louis, returned the call & suggested Dr Diaz rq an MRI of the lumbar spine, which she had already done after tests completed scans of lumbar/brain both unremarkablePt admitted to hospital under care ofNeal Coskun MD by Arshad Rehman MD who did not see patient until 6 or 7 PM (12 hrs after admitting pt). Dr Rehman¿s dx revealed gross tumors in 2 legs and pt una to move L leg on command. Dr. Rehman had a differential dx of poss epidural abscess and rq a stat MRI of cervical & thoracic spine. Una to do MRI secondary to change in mental statusPt transferred to 5th fl & care transferred to Dr Raymond who rq STAT MRI of brain, cervical & thoracic spine.There is no record of this order or why it was not carried outRN Cabading took over the shift and @ 7:30 reported to Dr Raymond that pt was hallucinating, pulling out IV, removing electrodes. Dr Raymond rq a radiology & neurology consult & vancomycinNurse Cabading gave pt the medicationNoted una to do MRI yesterday as uncooperative even w/vancomycin.Neurologist Dr Refaat El-Said MD saw pt and again rq MRI of thoracic, cervical W/WO contrast.Infectious disease specialist Dr Hsu saw pt and had initial impression of bacterial meningitis and rq epiduralMRI performed w/dx spinal abscess compressing spinal cord.Pt transferred to FL Hospital So.All this took place over 50 hours from the time patient walked into ER to final diagnosis.
