Final Diagnosis For Which Treatment Was Sought Including Patient's Actual Condition
The patient was admitted to the hospital on 8/20/02 with a history of severe peropheral vascular disease and had previously undergone a right below-knee amputation in April 2002 as a result of this disease. He was admitted to undergo a left femoral artery angioplasty.Postoperatively Mr. Poole did well although he did have a retroperitoneal hemorrhage and had to be transfused with two units of packed red cells.Dr. Stevenson was called in for pulmonary management. Mr. Poole tolerated the procedure well and postoperatively was placed on Aminophyline drip to assit with his asthma. Mr. poole continued to received Solu-Medrol 125mg IV every 6 hours postoperatively.He continued to do well from a pulmonary viewpoint and infectious disease physicians continued to manage his bacteremia.On September 5, 2002, the IV Solu-Medrol was discontinued after the 5:30 p.m. dose (the records do not reflect a physician's order for this).Four days later Mr. Poole suffered a cardiopulmonary arrest.He was revived after approximately 20 minutes of resuscitative efforts.He was placed on ventilator remained unresponsive until life support was withdrawn and he expired on 9/19/02.

Solu-Medrol was discontinued with no physician's order on the chart resulting in the patient's respiratory arrest and ultimate demise.
