The patient was seen 06 Oct 2017 with complaints of severe 10/10 pain headaches and sciatica. Low back pain was treated with Motrin and Lidoderm. In regards to the headache pain, the patient was advised to increase fluids, and alternate Motrin and Tylenol to control headache. Patient was released without limitations and to follow-up as needed.
