Patient presented to ER with left leg redness, pain and swelling. Patient weighed 350 lbs, had sleep apnea and many co-morbidities.

Patient was diagnosed with left leg cellulitis and sepsis. Vitals and labs were taken. Patient was administered antibiotics. the insured nurse documented the patient's initial vitals, that he was lethargic and that Phenergan was causing him to have confusion. The following morning the patient coded while in the shower.
