PATIENT SEEN DOCTOR IN APRIL 1998. THEREAFTER SHE SWITCHED HER PRIMARY CARE TO FRANK A THOMPSON WHO SAW HER FROM 1999 THRU OCT 2000. IT IS ALLEGED THAT THE PATIENT UNDERWENT DENTAL CLEANING AND WAS NOT GIVEN PROPHYLACTIC ANTIBOTICS DESPITE HER HISTORY OF AORTIC VAVLE REPLACEMENT IN 1992. PATIENT WENT ON TO DEVELOP PROSTHETIC VALVE ENDOCARDITIS WHICH CAUSED HER HEMODYNAMIC DETERIORATION AND CARDIAC ARRHYTHMIAS. PATIENT SUFFERED CARDIAC ARREST ON 11-20-00 AND DIED THEREAFTER.
