The patient was admitted to another hospital on 12/31/11 with an acute septic embolic CVA, septic lesions of foot as well as septic endocarditis allegedly due to failure to follow up on status of pneumothorax and premature hospital discharge. She also lost one of her toes to embolic event and necrosis requiring surgical amputation. Six months later, she underwent a mitral valve replacement.
