L5-S1 degenerative disc disease, L5-S1 herniated nucleus pulposus, left L4 right L4 and R L5 neuroforaminal stenosis. Central spinal stenosis at L3-4, L4-5, and L5-S1. Multi-level spinal stenosis at L3-4 and L5-S1, and mild central and bilateral lateral recess stenosis at L4-L5 level.

L5-S1 anterior lumbar interbody fusion and removal of pelvic mass

During retroperitoneal approach performed by Dr. Ramon Vazquez, he noted a pelvic mass and removed it as suggestive of gynecological malignancy. Pathology studies revealed presumed mass was patient's left vestigial kidney.

Improper removal of patient's pelvic kidney.
