Final Diagnosis For Which Treatment Was Sought Including Patient's Actual Condition
50 yo female with hx of Multiple Sclerosis, back surgery and pain management had complaints of right 5th toe pain x 2 months. Stenting and grafting were attempted but were unsuccessful.The patient developed coolness, numbness, severe critical ischemia and had absent Doppler signals.Repeat vasculariztion of the RLE was attempted without success.

On 1/24/2006 the patient underwent BKA with the application of immediate fitting prosthesis. The patient later also had debridement for a non-healing wound site.

It is alleged that the MD failed to order medical therapy such as aspirin or Plavix during the initial office visit of 12/15/2005 or the 12/19/2005 visit before the superficial artery angioplasty,that the performance of the superficial femoral artery angioplasty and stent was not an indicated procedure, andthe lack of the initiation of anti-platelet therapy after angioplasty and femoral popliteal bypass led to a Right below knee amputation
