Final Diagnosis For Which Treatment Was Sought Including Patient's Actual Condition
Dr. Castaneda provided uneventful and standard pre-natal care for the patient?s mother at his office. Dr. Castaneda?s only contact with the patient was at the hospital, he did not treat the patient at the clinic. Dr. Castaneda was responsible for managing the labor and delivery and oversee the certified nurse midwife for all clinic patients on September 18, 2004. The patient?s mother arrived at hospital at 6:30 am for pitocin induced delivery.
Operation, Diagnostic, Or Treatment Procedure Rendered Causing The Injury
The patient?s mother arrived at hospital at 6:30 am for pitocin induced delivery. Dr. Castaneda performed a vaginal exam at 5:55 pm the patient was dilated to 5cm and he ruptures the membranes. The NICU was notified by the nursing staff about an hour later that the membranes had been ruptured and there was slight meconium staining. Shortly after Dr. Castaneda saw the patient?s mother he contacted Certified Nurse Midwife Moraga and advised her that he was going to dinner and he asked her to oversee the labor and call him if needed. He is not obligated to stay at the hospital with either indigent clinic type patients or a patient he is treating under his PA as long as he is within 30 minutes of the hospital. Nurse Moraga charts at 6:53 pm and dilation is 5-6cm. At 8:30 she charts and the patient?s mother is completely dilated effaced that fetus is in a vertex position and -1 station. At 10:18 pm an increased amount of bloody show is noted with the baby at +1 station. At 10:38 nurse Moraga is present and preparing for delivery. The baby/patient is delivered at 10:51 pm there are no notes by nurse Moraga or the labor and delivery nurse of any problems with the delivery. It is noted in the neonatologist?s notes ?difficult shoulder delivery?. Dr. Castaneda returned to the hospital around midnight and was told by nurse Moraga that the delivery went smoothly with no problem. Dr. Castaneda says he saw the patient?s mother that evening and once or twice before being discharged by he never saw the baby and was never told of any problems until the receipt of the notice of intent.
