Presented to the Outpatient Center for a right diagnostic mammogram.
Operation, Diagnostic, Or Treatment Procedure Rendered Causing The Injury
Right diagnostic mammogram was performed on asyptomatic patient after abnormal screening mammogram.Physician evaluated the patient's diagnostic mammogram and spoke to her. Patient assured physician that the same area was evaluated in the past 2-3 years with benign results.Physician was assured the outside films were coming in the next few days.Physician gave the exam a bi-rads 0 conclusion (more imaging was needed) to be sure that if the prior mammogram did not arrive, the patient would be called back for more study.The mammogram did not come and the patient was not recalled by the mammographic office.

Patient returned to the Outpatient Center on 07/23/04 with a right breast mass and underwent a right diagnostic mammogram with a diagnosis of invasive poorly differentiated ductal adenocarcinoma, Grade 3, requiring a right mastectomy, chemotherapy and radiation treatment.
Severity Of Injury
Permanent: Significant - Deafness, loss of limb, loss of eye, loss of one kidney or lung.
