Final Diagnosis For Which Treatment Was Sought Including Patient's Actual Condition
15 year old with history of asthma seen by insured after admission for wheezing and imaging showing possible subglottic stenosis. Insured treated with fluids, broncho dilators and noted improvement and stability. At 11:00 am the following morning insured noticed patient had deteriorated through the night, blood gas reports showed insufficiency and patient was critical. He found patient with life threatening obstructions after sedation had been given against his advice.
Operation, Diagnostic, Or Treatment Procedure Rendered Causing The Injury
Insured was forced to perform emergency tracheotomy delayed by incomplete surgical tray and missing instruments with delay due to anesthesia staff needing to be called in. Patient demonstrated profound respiratory acidosis and profound hypoxemia after difficult tracheotomy intubation. Patient could not be resuscitated and died.
