MORRISON'S ASSESMENT AND PATIENT'S HISTORY AND PHYSICAL IN THE CHART CONSISTED OF UPPER GI BLEED, RULE OUT GASTRIC ULCERS, CHRONIC BRONCHITIS, ALCOHOL AND TOBACCO USE.THE GENERAL SURGEON PERFORMED AN EMERGENCY PARTIAL GASTRECTOMY ON 01/05/01 AND FOLLOWING THE SURGERY THE PATIENT WENT INTO RESPIRATORY FAILURE.
