PATIENT SUSTAINED A COMPLETE OCCLUSION OF THE RIGHT SUPERFICIAL FEMORAL ARTERY NECESSITATING A RIGHT BELOW THE KNEE AMPUTATION WHICH WAS FREE OF COMPLICATIONS.
Operation, Diagnostic, Or Treatment Procedure Rendered Causing The Injury
PATIENT WAS ADMITTED UNDER THE CARE OF THE INSURED WITH AN ADMITTING DIAGNOSIS OF INTESTINAL OBSTRUCTION, ARTRIAL FIBRILLATION, ANTHROPATHY AND MALIGNANT PROSTATE NEOPLASM. THE PATIENT STARTED TO DEVELOP A BRUISED LEFT FOOT WITH DISCOLORATION AND PAIN AND HIS TOES STARTED TO TURN BLUE/PURPLE IN COLOR AND COLD TO TOUCH.THE INSURED WAS ON THE OPINION THAT THE PATIENT WAS SUFFERING FROM RIGHT FOOT NECROSIS AND ORDERED AN ARTERIAL DOPLER ASAP, WHICH WAS DENIED BY THE PRIMARY PHYSICIAN.IT WAS ORDERED A THIRD TIME BY THE INSURED, BY THE TIME IT WAS NOTED THAT THE RIGHT FOOT GREAT TOE AND 2ND TOE WERE NOTED TO BE GANGRENED.THE PATIENT WAS REFERRED TO AN ORTHOPEDIC SURGEON AND A BILATERAL ANGIOGRAM WAS PERFORMED WHICH REVEALED COMPLETE OCCLUSION OF THE RIGHT SUPERFICIAL FEMORAL ARTERY AND A RIGHT BELOW THE KNEE AMPUTATION WAS PERFORMED.

THE INSURED'S THOUGHT PROCESS WAS THAT THERE WAS NO EVIDENCE TO SUGGEST AN ARTERIAL OCCLUSION BASED ON THE REGULAR RATES AND RHYTHMS ON ALL OF THE PATIENT'S HEART EXAMINATIONS THEREBY ELIMINATING THE IDEA OF AN ARTRIAL FIBRILLATION PATTERN.SHE NEVER FOUND THE PLAINTIFF'S FOOT TO BE ANYTHING BUT WARM AND ALWAYS NOTED POSITIVE PULSES ON EXAMINATION.SHE REQUESTED AN ARTERIAL DOPPLER TO BE DONE SEVERAL TIMES, BUT THE PLAINTIFFS PRIMARY PHYSICIAN DENIED THE STUDY AND AT THAT POINT SHE REMOVED HERSELF FROM THE PATIENT'S CASE.WHEN CONFRONTED WITH THIS SITUATION IN THE FUTURE SHE WILL SEND THE PATIENT TO THE EMERGENCY ROOM.
