On 4/29/2016, a the patient was seen in the ER for a cerebrovascular accident and admitted to the hospital.
Operation, Diagnostic, Or Treatment Procedure Rendered Causing The Injury
The ER physicians ordered consults to Physical Medicine and Rehabilitation and Cardiology. He also ordered a Head/Brain CT which showed atrophy with chronic small vessel ischemic changes. There was no evidence of an acute infarct or large territorial infarct or hemorrhage. The patient was discharged to rehab facility on the 5th by the attending physician who documented the MRI showed cerebral vascular accident and the TEE showed thrombus in the left atrial appendage, and that the pt. would start Plavix in 2 weeks from date of stroke as per cardiology. The patient was stable and accepted for acute rehab by our insured.
