Final Diagnosis For Which Treatment Was Sought Including Patient's Actual Condition
On 12/08/09, a 64 yom had a prostatectomy performed at St. Joseph's Hospital for prostate cancer. The pt had undergone aortic valve replacement in December of 2008. On 12/30/09 the pt's urologist noted the pt complained of fever and abdominal pain. He was ordered a CT of the pelvis which indicated mild sigmoid diverticulosis with no abscess identified. On 01/03/10 the pt presented to St. Joseph's Hospital Emergency Department with complaints of left arm pain, increased with movement, and flulike symptoms for a week. The pt was discharged with a diagnosis of Acute Tendinitis Left Shoulder with instructions to follow up with primary care physician in three days.
Operation, Diagnostic, Or Treatment Procedure Rendered Causing The Injury
The following day, on 01/04/10, the pt presented to his primary care physician, with complaints of fever, chills, night sweats and arthralgia. A diagnosis of fever was noted although never reproduced clinically and orders for ESA, FTE's blood cultures, urine for cultures/sensitivity and 2D ECHO. Antibiotics were not ordered, pending results or blood cultures. On 01/05/10, the echocardiogram was performed, results were interpreted by a cardiologist as non-concerning. On 01/06/10, blood work results were significant for cultures with gram positive cocci in cluster, rheumatoid arthritis, WBC 14.7, neutrophils 78 and lymph's 10. Orders were written by pt¿s primary physician for direct admission to St. Joseph's Hospital. Upon admission it was noted pt was alert and oriented x3 with stable vital signs. Pt was placed on telemetry and but had not complaints of difficulty in breathing, dizziness or weakness.

Later that evening pt's wife called out for help and the pt was found unresponsive but with respirations. The wife reported that after the pt finished eating he felt pressure in his chest. A rapid response/code blue was called and the Cardiopulmonary Resuscitation Flow Sheet documented time of arrest at 6:18 p.m., time of code team arrival 6:19 p.m and time of first intervention 6:20 p.m. The patient was intubated at 6:22 p.m. He died that evening.
Severity Of Injury
