Patient presented to insured with complaint of two ulcerations on the anterior aspect of her leg, which were treated conservatively by insured. She was ultimately lost to follow-up after the second visit. Patient went on to a right leg BKA. It is presumed patient would allege that insured should have ordered home healthcare herself rather than rely on the PCP to do so; however, the PCP insisted that all home healthcare and vascular testing requests went directly through him, and insured complied with these instructions.
