Patient first began treatment with our insured on 1/13/2015 with a history of HTN, obesity, kidney stones, alleged recent former smoker. This was to establish as a new patient. Patient was counseled regarding smoking, dietary changes, exercise, weight loss, and blood work was ordered. Patient returned on 1/15 after an unknown dental procedure with elevated BP and dizziness.
