Final Diagnosis For Which Treatment Was Sought Including Patient's Actual Condition | |||||
Patient underwent a CT scan of the maxillofacial skeleton without contrast which revealed complete left maxillary sinus opacification and thickening of the maxillary sinus walls, indicating a chronic process. Patient was scheduled for a balloon sinuplasty. | |||||
Operation, Diagnostic, Or Treatment Procedure Rendered Causing The Injury | |||||
On July 19, 2016, Dr. Patel, an otolaryngologist, performed a balloon sinuplasty on the left and right maxillary sinuses, left and right sphenoid sinuses and the left and right frontal sinuses on this patient. The procedure was uneventful. Following the sinuplasty, Dr. Patel observed his patient to ensure that no complications were incurred during the procedure. There were no complications noted. No drainage or bleeding was observed and no complaints of headache from the patient. The patient was discharged to home with instructions to follow-up if he developed any potential complications including cerebrospinal fluid drainage, bleeding and significant headaches.The following day, July 20th, Dr. Patel's office staff contacted the patient to follow-up and see how this patient was doing after the procedure. The staff member reported that the patient was doing well. On 7/25/2016, the patient and his wife were driving and the patient became aphasic. The patient was taken to the emergency room where he was considered a "stroke alert". As a part of the stroke alert workup, and in order to determine whether or not the patient was a potential TPA candidate, a CT scan of the head was performed and interpreted by the radiologist. The radiologist's written report did not reflect any evidence of intracranial aneurysm or vascular thrombosis. However, he identified intracranial gas densities along the upper portion of the head. These findings were communicated directly to the ER physician. Also reported by the radiologist was the note stating "I can't exclude a non-displaced fracture of gas-producing infection". As a result of the "stroke alert" status, a neurologist felt that TPA was an appropriate medication to administer. The patient was provided TPA and did, unfortunately, have a small intracranial hemorrhage. on 7/31/2016, a CT of the maxillofacial skeleton with contrast was performed which revealed an absence of the left fovea ethmoidalis/cribriform plate. | |||||
