Patient presented to the insured on 1/20/09 with complaints of pain associated with the posterior heel of 3 months duration.Prior treatment had consisted of physical therapy, but without improvement.Radiographs were said to depict no posterior spur, yet there was a small nodule that was palpated bilaterally.The patient was diagnosed with Achilles tendinitis, retrocalcaneal bursitis, and gastrocnemius equinus.Each heel was injected with 40mg of Kenalog.He was given a heel raise, Tylenol was recommended, and Flector patches were prescribed.The patient was to discontinue use of stretching and the night splint.On 2/3/09 the patient was doing well and had no pain; however, he presented again on 4/27/09 with recurrent symptoms.Radiographs were made again and this time noted to demonstrate retrocalcaneal spurs bilaterally.A positive tinel sign was noted with percussion over each heel.Cryotherapy was discussed for the retrocalcaneal nerve and pathologic tissue.The procedure was performed on the left heel on 5/4/09, and on the right heel on 5/7/09.After each procedure, the heels were injected.Patient subsequently left the state and went on to treat with another doctor who diagnosed him with bilateral Achilles tendon tears and performed surgical repair.Patient alleges the injections caused the Achilles tears, and that insured was negligent in injecting him.
Severity Of Injury
Temporary: Major - Burns, surgical material left, drug side effect, brain damage.Recovery delayed.
