Patient was first seen by insured on 5/6/11 for an ulceration of the fifth toe left, foot.It was determined that the patient had gangrene of the tip of his 5th toe along with a paronychia of his left hallux.Insured felt that the patient had significant arterial insufficiency.X-rays were ordered, the area debrided and Keflex prescribed.He also ordered non-invasive vascular testing and a consult with vascular surgeon.Patient was seen on a number of occasions between the initial visit and June 27.Throughout this treatment there are various and multiple ulcers noted on the foot with a relatively large heel lesion being described on 6/13.It appears that insured abdicated the overall control of the patient to the vascular surgeon as he had diagnosed the PAD and had the patient referred to the proper specialist.There was nothing insured could do for the PAD, which was the underlying cause of the ulcers; however, he did continue to see the patient and render wound care, including debridement, possibly whirlpools and dressing changes. At some time before the final visit, patient apparently went on a trip to Indiana for a week and, presumably, walked excessively.By the time he returned, he was found to be significantly infected.The patient was eventually admitted to the hospital and, on 7/21, underwent a BKA.He alleges insured failed to timely refer him to a vascular specialist and failed to appropriately treat the wounds.
