The patient was under the care of the physician from December 1998 through December 2003.The patient's history included diabetes mellitus, gastrointestinal reflux disease, hypertriglyceridemia, hypercholesterolemia, and insomnia.Most of the visits were for routine care, with the patient offering no specific complaints.In May 2002 bronchitis was diagnosed.In January 2003, thoracic spine films, ordered for left neck, back, and shoulder pain, showed arthritis only.In January 2003, the patient was diagnosed with fibromyalgia.In July 2003, the patient was seen for a nonproductive cough and sore throat; she was diagnosed with pharyngitis.In August 2003, the patient had a routine office visit with no complaints.In November 2003, the patient was diagnosed with bronchitis and in December 2003, she was seen for continued history of coughing and shortness of breath.On December 14, 2003, an emergency room visit for recurrent chest pain resulted in a chest x-ray which demonstrated a mass in the lung.The patient was subsequently diagnosed with stage IV non-small cell lung cancer with metastasis throughout her body.
