Final Diagnosis For Which Treatment Was Sought Including Patient's Actual Condition
The insured treated the patient in the offices of Coast Dental from 12/31/2003 to 5/5/2004, a total of 4 full months. She wanted partials replaced with fixed bridges.She was noted as having 30% recession and 35% Bone loss and TMJ on the rt. and a Class II malocclusion. She had not been to a dentist in the prior 10 years.
Operation, Diagnostic, Or Treatment Procedure Rendered Causing The Injury
The insured treated the patient in the offices of Coast Dental from 12/31/2003 to 5/5/2004, a total of 4 full months. She wanted partials replaced with fixed bridges.She was noted as having 30% recession and 35% Bone loss and TMJ on the rt. and a Class II malocclusion. She had not been to a dentist in the prior 10 years.We completed an oral exam, X-rays, full mouth debridement prior to comp exam, perio scaling and root planing, 2 bridges 6-11 & 13-15 and the remainder of the (7) teeth in the maxillary arch were crowned. On the patient's last visit, we referred her to an endodontist for evaluation of #4& 5. She never returned to our care.

Dr. Pham failed to provide a proper comprehensive diagnosis with adequate radiographs, study models or impressions, periodontal examination and depth charting prior to initiating crown and bridge restorations in the plaintiff?s mouth; Dr. Pham failed to properly diagnose, document and/or treat lesions which were reflected on x-rays taken in the plaintiff?s mouth prior to undertaking major restorative work on plaintiff?s mouth;Dr. Pham failed to properly diagnose, document and/or perform any needed root canal therapy prior to undertaking major restorative work in plaintiff?s mouth;
Principal Injury Giving Rise To The Claim
All resulted in extensive restorations having to be re-done after patient developed erosive lichen planus due to cheek biting.

Temporary: Minor - Infections, misset fracture, fall in hospital. Recovery delayed.
