Diagnostic Information

Final Diagnosis For Which Treatment Was Sought Including Patient's Actual Condition
On 08/21/10, a 19 yof presented to St. Anthony's Hospital Emergency Department with complaints headache. She had been evaluated at SAH ED in April of 2010 for the same symptoms, but left without treatment.
Operation, Diagnostic, Or Treatment Procedure Rendered Causing The Injury
On 08/21/10, the patient's neuro exam was documented to be within normal limits and no fever. She was treated with Toradol,improved and discharged. On 08/22/10, the patient returned to SAH ED with complaints of abdominal pain and vomiting. The patient informed the nursing staff that she still had a headache not relieved from the day before, but denied headache to the physician on this visit. She was treated with a liter of IV fluid and Zofran. The patient's neuro exam continued to be normal and her nausea/vomiting resolved. She was discharged home. The following day the patient was found unresponsive at home and taken to Bayfront Medical Center where she expired from a massive cerebellar stroke.
