Diagnostic Information

Final Diagnosis For Which Treatment Was Sought Including Patient's Actual Condition
On June 9, 2006, the patient was admitted to the hospital with complaints of lower back pain, loss of appetite, and weight loss with history of osteoporosis. Nursing evaluation indicted the patient was frail, cachexic and in severe discomfort. She had a 10cm decubitus ulcer State II on the right side of her back. The patient was transferred to the nursing home where the insured D.O. was employed for physical and occupational therapy.
Operation, Diagnostic, Or Treatment Procedure Rendered Causing The Injury
The patient's weight was 76 lbs and has impaired physical function, malnutrition, depression and cognitive impairment. The insured D.O. had determined the patient had failure to thrive. On June 22, 2006a nurse was in the process of placing a Foley catheter when the patient went into respiratory distress and became unresponsive. The Fire Rescue was called and transported the patient to the hospital.
