This claimant, with a complex medical history was admitted to the hospital in July 2004 with a diagnosis of fever, shortness of breath, deep venous thrombosis, pneumonia and various problems related to anorexia and weight loss.With a concern for sepsis from an existing line, the admitting physician wrote an order for the department of anesthesiology to place a new central line.As the on-call anesthesiologist, Dr. Wisely responded to the request and unsuccessfully attempted to place an internal jugular and subclavian line on the left side.Dressings were applied with a plan to attempt placement the following morning.Some three hours later, the patient had a sudden change in condition with shortness of breath which progressed to a cardio pulmonary arrest.Resuscitative efforts, managed by other health care providers were unsuccessful and the patient expired.A post insertion chest x-ray was negative for pneumothorax or hemothorax.An autopsy revealed numerous medical problems including a laceration of the left subclavian artery with subcutaneous hematoma.Bleeding is a recognized complication of central line insertion and was covered during the pre procedure informed consent.Supportive expert review was obtained.To avoid the uncertainty of proceeding to a jury trial, with no admission of liability, the matter was resolved during mediation in February 2009.
