Final Diagnosis For Which Treatment Was Sought Including Patient's Actual Condition | |||||
The patient first began seeing his PCP, Dr. Salansky, in her Family Practice clinic beginning in 2010. He reported a history which included depression, anxiety, asthma, dyslipidemia, high blood pressure, testicular pain with cyst, ED, vasectomy, arthritis, migraines, left lung nodule, and low back pain. He was not consistently compliant with his diet and did not exercise regularly. He smoked approximately 20 cigarettes per day and had no interest in quitting. Over the next six years and up to the time of his death in 2016, Dr. Salansky was following the patient for his many conditions including his blood pressure, while recommending lifestyle changes to improve his weight, stress levels and abnormal cholesterol levels. When the patient was compliant in following these lifestyle recommendations his overall condition showed improvement, however, he was often non-compliant. On December 29, 2016, shortly after returning from a long out-of-state driving trip, EMS was called to the patient¿s workplace after it was reported he had sudden onset of chest pain and shortness of breath. He suddenly became unresponsive and despite resuscitation efforts, he went into asystole and passed away. | |||||

Outpatient office visits from 2010-2016. During this time the patient presented with many chronic and significant problems, not just high blood pressure or high cholesterol and therefore, Dr. Salansky was not treating him for each of his conditions at every single visit. Her primary approach as to the high blood pressures and abnormal cholesterol, except during the approximate year prior to his death wherein his blood pressure medications were modified, was to manage those issues with lifestyle changes and recommendations. He was not on a statin at the time of his death. | |||||

The patient likely suffered a cardiac event and died. It is notable that the patient did not specifically complain of cardiac symptoms prior to his death, including chest pain, arm tingling, etc. EKG taken in the office in April of 2016 was within normal limits and Cardiology work up in 2010 was normal as well. According to the Death Certificate, the patient¿s cause of death was listed as cardiac arrest and hypertension. Additionally, other contributing factors to the cause of death were asthma, rheumatoid arthritis, cervical spondylosis, dyslipidemia, and tobacco abuse. The Death Certificate indicates that tobacco use ¿probably¿ contributed to his death. No autopsy was performed. | |||||
