Patient referred for lower abdominal pain after being seen by her PCP where pelvic ultrasound had been done showing the left ovary as hyperechoic and partly cystic that may represent physiologic cyst with hemorrhage but neoplastic process could not be excluded.

At the time of presentation to Dr. Hernandez the patient was asymptomatic and he gave an order for repeat ultrasound. She did not report a past family history of ovarian carcinoma at that time. He did note the small cyst found on the prior ultrasound. The patient never had the ultrasound as ordered. Two years later she went for trans-vaginal ultrasound and an ovarian mass was reported.
