Final Diagnosis For Which Treatment Was Sought Including Patient's Actual Condition
Lou Anne Bruckner presented to Florida Hospital-Altamonte on November 15, 2009. She complained of lower extremity weakness with a history of prior thoracic spine surgery in 2007. A neurosurgeon consultant was called, but refused to take the consultation. As a result, Mrs. Bruckner was transferred to Florida Hospital-Orlando, where she underwent thoracic surgery. She was admitted for 9 days and Mrs. Bruckner was then discharged to Florida Hospital-Winter Park for rehabilitation.
Operation, Diagnostic, Or Treatment Procedure Rendered Causing The Injury
Dr. Lalli evaluated Mrs. Bruckner on November 19, 2009. He noted Mrs. Bruckner had no lower extremity strength post a thoracic spine surgery. However, the doctor noted Mrs. Bruckner was moving her right foot, which was an improvement. Dr. Lalli also ordered multi podus boots. The following day. Dr. Lalli evaluated Mrs. Bruckner. He felt Mrs. Bruckner could improve with physical therapy bedside. He requested Dr. Jenkins to okay the activity. Dr. Jenkins indicated the patient was now able to sit upright in bed and could participate in therapy. Dr. Gebauer read the chest MRI ordered by Dr. Lalli, x-ray was negative. On November 21, 2009, Dr. Lalli ordered a physical therapy consultation to determine where Mrs. Bruckner would receive physical therapy after her discharge. Dr. Lalli evaluated Mrs. Bruckner on November 24, 2009. He indicated the patient was stable and the plan was impatient rehabilitation when Dr. Jenkins agreed to discharge the patient. Dr. Lalli also mentioned he wanted to start Lovenox, an anti-coagulant, when approved by Dr. Jenkins. In fact, Dr. Lalli wrote a physician¿s order to the nurses to ask the neurosurgeon when it would be okay to start Lovenox. Dr. Jenkins wrote in the order section that the patient was not to receive Lovenox therapy. This note was made after Dr. Lalli¿s request, so it appears Dr. Jenkins received the request and denied it. The following day, at 2:15 p.m., Dr. Jenkins noted it was okay to start Lovenox and he discharged the patient to Florida Hospital-Winter Park for rehabilitation. Mrs. Bruckner was transferred to Florida Hospital-Winter Park at 4:30 p.m. It appears Dr. Lalli did not evaluate Mrs. Bruckner during that short window on November 25, 2009, prior to her discharge to Florida Hospital Winter Park. He did not write any additional orders in the Florida Hospital-Orlando chart.
