Final Diagnosis For Which Treatment Was Sought Including Patient's Actual Condition
On March 9, 2008, I assumed care of a patient admitted in early pregnancy for evaluation of abdominal pain and possible ectopic pregnancy. The patient had abnormally rising hCG levels (26.7% rise in 48 hours), an abnormal pelvic ultrasound not consistent with her elevated hCG level, and worsening pelvic pain. The patient was offered options of expectant management, surgical intervention with laparoscopy, or medical management with MTX for presumed ectopic pregnancy. After receiving counseling on the risks and benefits, the patient opted for MTX treatment. The patient was later found to have an intrauterine pregnancy. Despite counseling by Perinatology to terminate the pregnancy, the patient decided to maintain her pregnancy and delivered an infant with multiple anomalies, none of which were known to be associated with MTX administration, but were more consistent with VACTERL association.
