57 yo female presented with pelvic pain , relaxation of the pelvic musculature, 2nd-3rd degree rectocele, 1st- 2nd degree cystocele and 1st degree uterine prolapse.

Total vaginal hysterectomy with bilateral salpingo-oophorectomy, anterior colporrhaphy, posterior colporrhaphy and repair of enterocele for pre and post op diagnosis of pelvic relaxation and 2nd and 3rd degree rectocele, cystocele, and 1st degree uterine prolapse. No vaginal tissue was removed per the pathology report

Post op the patient had complaints of dyspareunia and pelvic pain.CT done on 2/9/2010 demonstrated vaginal shortening and the appearance of vaginal agglutination, however, this was non-specific. Plaintiffs alleged that too much vaginal tissue was removed during surgery causing permanent vaginal defect.
