The patient was referred to urology for history of prostate cancer. Urologist ordered ultrasound that showed large solid renalcomplex mass. Patient was called and advised of finding. His urology appointment was rescheduled to an earlier date. Patient was also referred to oncologist. The patient missed several appointments with urologist. These events occurred while there was a transition to an electronic records system. The patient also had a change of PCP for one year
