Final Diagnosis For Which Treatment Was Sought Including Patient's Actual Condition
During the prenatal period, deceased claimant was determined to be high risk due to having a fetus with multiple cardiac anomalies, followed by development of hydrops. At 32 weeks pregnant, claimant presented to OB emergency room at Sacred Heart with complaints of no fetal movement. An ultrasound confirmed loss of fetal heart tones. Claimant was induced and delivered a stillbirth by her primary OB. Complications occurred postpartum, where patient received treatment at Sacred Heart by the oncall OB, Dr. McCutcheon, in addition to Sacred Heart nursing staff, and Sacred Heart Hospitalists. Claimant's complications included postpartum hemorrhage, severely low potassium, and hypoxia. Claimant ultimately went into PEA arrest after intubation and failed resuscitation efforts. The causeof death was determined to be cardiopulmonary arrest.
Operation, Diagnostic, Or Treatment Procedure Rendered Causing The Injury
On the date of incident, claimant had received treatment for a postpartum hemorrhage and hypoxia thought to be opioid induced including wearing a non-rebreather mask. Dr. McCutcheon was called to Sacred Heart to address post-delivery uterine bleeding complications. Dr. McCutcheon arrived at bedside, performed a uterine sweep, gave additional medications for bleeding, andordered a CTA for patient's hypoxia that was noted. Because patients repeat blood count was unchanged and clinical picture was discordant to earlier findings, Dr. McCutcheon also ordered 2 units of blood and repeated labs. After talking with the patient and leaving her bedside to resume on call shift duties, Dr. McCutcheon was called by Sacred Heart OB nursing staff and notified of claimant's drop in potassium level and patient's complaints of chest pain. Dr. McCutcheon ordered potassium 40 meq po and requested status update of CTA and EKG results. Prior to Dr.McCutheon's receipt of the CTA results and administration of potassium replacement, claimant went into PEA arrest and was intubated by Sacred Heart rapid response team and Sacred Heart hospitalists who attempted to resuscitate claimant. The claimant coded and cause of death was determined to be cardiopulmonary arrest.

Involved providers completed online courses on the recognition and treatment of pulmonary edema mirror syndrome. Also, at Sacred Heart Hospital where the incident occurred we were notified their staff instituted Rapid Response rounding and multidisciplinary simulation labs for events like this.
