patient presented to hospital on 8-21-2014 with abdominal complaints. Image testing revealed a perforated ulcer

Patient was taken to surgery to repair duodenal ulcer by physician. Following repair scrub tech and nurse confirmed correct sponge count x 2.

Misdiagnosis Made, If Any, Of Patient's Actual Condition
8 days following surgery, patient returned to physician with reported abdominal discomfort and nausea. CT scan showed fluid collection. Patient was taken back to surgery on 9-9-2016.
Principal Injury Giving Rise To The Claim
During the 9-9-2017, it was discovered a lap sponge had been left from the original surgery despite the nurses sponge would indicated the sponge count was correct x 2.
Severity Of Injury
Temporary: Major - Burns, surgical material left, drug side effect, brain damage. Recovery delayed.
