| 69 year old male presented to the MD Now Urgent Care on 3.19.2017 with a dry cough and chest congestion for the past 3 days. Dr. Queiroz prescribed an antibiotic. Because of the patient¿s cardiac history he ordered an EKG to obtain a baseline for potential changes. He found nothing alarming in the EKG. | |||||

| The EKG was never scanned into the chart by support personnel. The doctor¿s interpretation was hand written on the EKG strip | |||||

| The doctor ordered an EKG. Several hours later the patient presented to the Emergency Room in cardiac arrest. The cardiac catheterization was negative for MI and it was felt the patient had suffered from a change in rhythm secondary to low potassium. Under the procedures at the MD Now Medical Center, after the EKG was performed and interpreted by the physician, the staff at MD Now should have scanned the EKG into the medical record. This was not done. He therefore was left in a situation where the center where he was employed failed to properly maintain a medical record and he did not have the medical record available to him. The fault in this instance is that of the medical staff, which is not employed by the practitioner but rather employed by MD Now Medical Centers, Inc., which is also the employer of Dr. Queiroz. | |||||
